Target controlled infusion of remifentanil and propofol in elderly patients undergoing nasalendoscopy surgery

Abstract

OBJECTIVE To evaluate the efficacy and side effect during target controlled infusion of remifentanil and propofol in elderly patient undergoing nasal-endoscopy surgery.METHODS 60 patients of nasal-endoscopy surgery were devided into two groups based on age(non-elderly group age60y,and elderly group age≥60).All patients received remifetanil and propofol for induction and maintenance of anesthesia. The average target concentration of remifentanil and propofol were assessed,hemodynamic data were collected,and the recovery time was recorded in both groups of patients.RESULTS The Plasma Concentration of propofol at loss of consciousness of elderly group was significantly lower than that of nonelderly group,and the patients in elderly group also required target plasma concentrations of propofol and remifetanil lower than those in non-elderly group during anesthetic maintenance.Systemic blood pressure decreased significantly in both groups after induction and remained stable during surgery.The changes of diastolic blood pressure and heart rate were not statistically different at different points or between the two groups. The recovery time was prolonged in elderly group. CONCLUSION Carefully titrating the target remifentanil and propofol concentration enables the hemodynamic and anesthesia depth of the elderly patients be maintained within a similar range of those of the younger ones.
