Abstract

Objective To observe the effect of different insulin medication on urine albumin in patients with diabetic nephropathy. Methods The 98 patients with daibetic nephropathy were randomly divided into insulin pumps group( Group A) treated with continuously intradermal injection of novogen, novopen group(Group B) treated with novogen injection before meal and oral drug group(Group C) treated with oral repaglinide 1~2mg tid for a course of four weeks, then the urine albumin secretion rate (UAER) and creatinine(Cr)of the three groups in 24 hours were determined before and after therapy. Results The levels of UAER in Group A was significantly difference before and after treatment (P0.001), but the number of cases with UAER less than 200μg/min was dignificantly increased (P0.05). There no significnat difference in Cr levels were observed before and after treatment (P0.05),but the number of cases with Cr less than 100μmol/L was significantly increased (P0.05). In group B, there is no significant difference in the level of UAER before and after treatment (P0.05), but the number of cases with UAER less than 200μg/min was significantly increased(P0.05). In Group C, there was no differenced in UAER、Cr and these cases change(P0.05) before and after treatement (P0.05). Conclusion The level of urine albumin in diabetic nephropathy cases is reduced after intensive insulin treatment. But drug can reduce the level of ambumin in diabetic patients with nephropathy.
