Diagnosing serum cystatin-C , β2-microglobulin and microalbuminuriain early impairment of renal function in patients with chronic heart failure

Abstract
Objective: To investigate the diagnose value of cystatin-C(Cys-C),β2-microglobulin(β2-MG)and microalbuminuria(mAlb)in early impairment of renal function with chronic heart failure.Methods: Thirty six patients with chronic heart failure(CHF) were subdivided into four groups according to NYHA cardiac function classification criteria.In another set of experiment,another 13 patients without CHF and renal functional impairment were set as control.Blood samples were taken to measure serum Cys-C,β2-MG and mAlb by immunoturbidimetry.Results: No significant difference were found in age,blood routine,liver function and glucose.The levels of serum Cys-C,β2-MG and mAlb were enhanced,along with the increased level in the NYHA cardiac function classification,especially for NYHA-Ⅳ(P0.01).But no statistically significant difference was found among them in the level of serum creatinine and urea nitrogen.Compared to control group,the levels of serum β2-MG and mAlb all significantly increased from NYHA-Ⅲ and Ⅳ(P0.01),whereas the level of serum Cys-C started increasing on NYHA-Ⅱ(P0.01).And the estimated glomerular filtration rate(eGFR) significantly decreased on NYHA-Ⅱvia serum creatinine(P0.05).β2-MG and mAlb had positive correlations with Cys-C(P0.01) while β2-MG had positive correlation with mAlb(P0.01).Conclusion: The levels of serum Cys-C,β2-MG and mAlb can be used to detect early renal functional impairment of CHF with the levels of serum Cys-C reflecting renal functional impairment earlier.
