Detection of blood indexes for predicting the prognosis of acute pancreatitis

Abstract

Objective To provid valuable indexes for predicting the prognosis by investigating the correlation between blood indexes and severe acute pancreatitis.Methods A total of 139 patients with acute pancreatitis including 107 cases of mild acute pancreatitis(MAP group) and 32 cases of severe acute pancreatitis(SAP group),and another 42 patients with acute abdominal inflammation but not hepatobiliary or pancreatic diseases(control group) were detected and compared the clinical indexes as calcium ion,C-reactive protein,haematocrit,white blood cell count,prothrombin time,activated partial thromboplastin time,etc,on the 1st,3rd,5th,7th day and before the obvious deterioration of disease.Results Compared with control group and MAP group,the levels of the above indexes in SAP group were significantly different(P0.01),and the change of indexes lasted longer.The rate of accuracy of predicting severe acute pancreatitis was up to 87.2% when C-reactive protein was over 200 mg/L in 4 days after the attack of the disease.The total accuracy rate of predicting severe acute pancreatitis was up to 68.1% when Ca2+ was lower than 2.0 mmol/L,C-reactive protein was over 200 mg/L,and haematocrit was over 44.0% for male and 37.0% for female.Conclusion The dynamical determination of blood indexes,such as Ca2+,C-reative protein and haematocrit helps to diagnose severe acute pancreatitis,which can be used as indexes for the early prediction and diagnosis of severe acute pancreatitis.
