Association of uric acid bilirubin and high sensitive C-reactive protein with severity of coronary artery lesion

Abstract

Objective To investigate the association of uric acid,bilirubin and hs-CRP with coronary lesion.Methods Four hundred and twenty nine patients who had underdone coronary angioplasty and completed clinical examination of TG,TC,LDL-C,HDL-C,BUN and SCr were divided into the normal vessel group,scleratheroma group(coronary stenosis 50%) and the single vessel disease group(with only one ressel stenosis ≥50%) according to the angiographic results.All the patients in each group were further devided into the high LDL-C and low LDL-C subgroups.Results Higher level of UA was found in the single vessel disease and in the multi-vessel disease group compared with the normal vessel group(365.27±96.69 μmol/L vs 285.59±34.45 μmol/L and 385.67±74.00 μmol/L vs 285.59±34.45 μmol/L,both P0.05).The levels of direct bilirubin were lower in the single vessel disease group and in the multi vessel disease group compared with the normal vessel group(3.57±1.40 μmol/L vs 4.84±1.16 μmol/L and 2.99±1.45 μmol/L vs 4.84±1.16 μmol/L,both P0.05).No significant difference was found in indirect bilirubin level among the 4 groups.The levels of hs-CRP were higher in the single vessel disease group and in the multi-vessel disease group compared with the normal disease group(0.616±0.776 mg/L vs 0.164±0.432 mg/L and 0.934±0.826 mg/L vs 0.164±0.432 mg/L,both P0.05).The level of hs-CRP in the multi-vessel disease group was also higher compared with the single vessel disease group(0.934±0.826 mg/L vs 0.616±0.776 mg/L,P0.05).Single factor logistic regression showed that uric acid,direct bilirubin and hs-CRP are all correlation factors coronary artery lession while uric acid had the highest OR value(OR 5.347,95% CI 3.365-8.495).Advanced multi-factor logistic regression showed stronger correlation of uric acid and hs-CRP with coronary atery lesion compared with direct bilirubin(r=1.124 and 2.113,P=0.001 and 0.000,respectively).Conclusion Uric acid and hs-CRP increase along with aggravation in the severity of coronary lesion but indirect bilirubin does not show such correlation.
