Abstract
Objective To probe into the method of subclavian vein puncture and intubation,seek optimal site of puncture to improve the success rate of one-time puncture.Method Applying new site of puncture in the Improvement Group-Puncture at the cross area between the lower edge of the middle of clavicle and 0.5~1 cm away from 1cm above the second rib edge,the needle aimed to the upper notch of the sternoclavicular joint bone,slowly and steadily advance the needleunderneath the clavicle into the depth of 3~4 cm,intubation depth:12 ~15 cm;While applying traditional puncture site-Subclavian vein puncture in the Traditional Group:Puncture at a distance of 0.5~1 cm from the lower edge of the middle of clavicle,needle aimed to the sternoclavicular joint,intubation depth:12~15 cm.Follow-up assessment was conducted on the success rate and complications.Result Among 45 cases in the Improvement Group,44 cases were successful with one-time puncture,1 case was successful with a second time puncture,puncture success rate was 100%;while among 45 cases in the Traditional Group,29 cases were successful with one-time puncture,10 cases were successful with a second time puncture,6 cases failed,puncture success rate was 86.67%.There's an obvious difference between the groups on puncture success rate(χ2=4.4643,P0.05). Conclusion Improved subclavian vein puncture is simple to locate,lock in place,nurse and operate with high success rate of one-time puncture,which will not affect the movement of the patients'neck,upper and lower limbs and deserves clinical application.
