Abstract
Objective To analyze the main factors of ventilator weaning of the patients who have received long-term mechanical ventilation(≥7 d)in ICU and to discuss the method of ventilator weaning in long-term mechanical ventilation patients.Methods The study group consisted of 45 long-term mechanical ventilation patients.The general condition,early trachea incision,primary diseases,the vital signs before ventilation,accessory examination and acute physiology and chronic health evaluation Ⅱ(APACHEⅡ) were collected.All the patients were divided into two groups according as the ventilator weaning was successful or failed.Comparative analysis was done in two groups of enacting clinical data.Results Ventilator weaning was successful for 23 cases,and failed for 22 cases.APACHEⅡ scores were much higher in failed group than in successful group,and there were obvious differences between two groups(P0.01).The ratio of tracheotomy was significantly higher in successful group than in the failed group(P0.05). The albumin and ventricular function in successful group were much better than those in the failed group.Conclusion Aggravating primary diseases and severe complication are the main causes of lower success rate of ventilator weaning.The basic condition before the mechanical ventilation,heart function and APACHEⅡ score can predict the success or failure of weaning.
