Abstract
Objective To evaluate the risk factors,distribution pattern,and prognosis of cervical squamous cell cancer with cervical lymph nod metastasis at primary diagnosis after hysterectomy.Methods Clinical data of stage ⅠB~ⅡB cervical carcinoma patients with cervical lymph nod metastasis and cervical squamous cell cancer cases without distant lymph node metastasis who had undergone radical hysterectomy and Bilateral pelvic lymph adenectomy for analysis at Xinjiang Tumor Hospital between 2003 and 2009 were analyzed.Results In 546 cases of ⅠB~ⅡB squamous cervical carcinoma after operation of cervical lymph node metastasis in 18 cases,transfer rate of about 3.3%.Multivariate analysis is revealed that the tumor size ≥4 cm,positive iliac lymph node and cancer embolus were significant risk factors of cervical carcinoma with distant lymph nod metastasis after hysterectomy.Serum level of squamous cell carcinoma a antigen(SCC-Ag) 2 μg/L before treatment,stromal invasion ≥1/2 and cervical lymph node metastasis were significant prognostic factors of cervical squamous cell carcinoma.Neck lymph node metastasis of cervical squamous cell carcinoma patients 3 year survival rate of 43.3%,lower than that of control group 95.2%(P0.05).Conclusion Cervical lymph node and Supraclavicular lymph node metastases are both distant lymph node metastasis sites for patients with squamous cancer of the cervix.Tumor diameter ≥4 cm,cancer embolus and positive iliac lymph node were significant risk factors of cervical carcinoma with distant lymph nod metastasis after hysterectomy.
