Estimation of Ventricular Diastolic Function by Spectral Doppler Echocardiography in Patients with Coronary Artery Diseases

Abstract

Objective: To observe the changes of ventricular diastolic function between the healthy and the patients with coronary heart disease by Doppler echocardiography. Methods: The study population included 61 patients with coronary heart disease. They were divided into two groups according to EF measured by echocardiography:one group with normal EF (EF≥50%,N=33)( group I ),the other abnormal EF (EF50%,N=28)(groupⅡ). The control group include 32 normal subjects. The pulsed Doppler was used to measured diastolic functional parameters of mitral and tricuspid flow. Diastolic functional parameters of pulmonary venous flow also be measured. Left ventricular end-diastolic pressure(LVEDP) was recorded with poly-lead physiologic recorder. Results: groupⅠ,both left and right ventricle diastolic function mostly presented relaxation damaged. groupⅡ,left ventricle diastolic function mostly presented pseudonormal pattern. PVad-Ad is correlated significantly with LVEDP (r=0.72,P0.01). PVad-Ad0ms predicts pseudonormal pattern of mitral flow with a sensitivity of 92% and a specificity of 80%. There was a remarkable correlation between left and right ventricular diastolic function(E/A: r=0.46,DT: r=0.54,P0.01). Conclusions: In the early coronary heart disease when left ventricle systolic function was normal,both left and right ventricle diastolic function mostly presented decrease. Left ventricle diastolic function will be further decreasing when the systolic function was abnormal. To use PVad-Ad can differentiate accurately pseudonormal pattern of mitral flow from normal pattern. Significant correlation was observed between left ventricle and right ventricle.
