Abstract
OBJECTIVE To study the influence of mannatide injection on immunities of gallbladder cancer patients and prevention of postoperative infections so as to provide guidance for clinical treatment.METHODS A total of 160 cases of gallbladder cancer,who were treated in the hospital from Jan 2011 to Jan 2013,were enrolled in the study and randomly divided into the experimental group and the control group,with 80 cases in each,then the experimental group was treated with mannatide injection from 3 days before surgery through 7 days after the surgery,while the control group was given the equal amount of glucose,both groups received the conventional use of antibiotics for treatment of infections;the clinical efficacy,incidence of postoperative infections,the changes of levels of inflammatory cytokines such as interleukin-2(IL-2),interleukin-6(IL-6),interleukin-10(IL-10),tumor necrosis factor-a(TNF-a),and the changes of levels of endotoxin,T lymphocyte subsets CD3+,CD4+,CD8+ and NK cell were observed and compared between the two groups.RESULTS Seven days after surgery,the levels of IL-10,TNF-a,and endotoxin of the experimental group were significantly reduced(P0.05),while the levels of IL-2 and IL-10 were significantly elevated(P0.05);the levels of IL-6 and TNF-a of the control group were significantly decreased(P0.05);as compared with those before the surgery,the indicators of the experimental group did not change significantly after the surgery;the levels of CD3+and CD4+ were reduced,the level of CD4+/CD8+ elevated,the level of NK cell decreased.Four days after surgery,the level of CD3+o f the experi mental group was elevated,the levels of CD8+ and CD4+/CD8+ decreased;the levels of CD3+ and CD4+ of the control groups were reduced,the level of CD4+/CD8+ elevated,the level of NK cell decreased.Seven days after surgery,the levels of CD3+、and CD4+ of the experimental groups were elevated,the levels of CD8+、and CD4+/ CD8+ decreased,the level of NK cell elevated;the levels of the indicators of the control group did not change significant,as compared with the control group,the levels of the CD3+,CD4+,and NK cell of the experimental group were elevated after the surgery;4 or 7 days after surgery,the levels of CD3+、and CD4+ were elevated,the level of CD8+ decreased,the level of NK cell elevated;the infection rate was 7.5% in the experimental group, 22.5% in the control group;as compared with all the data above,the differences between the two groups were statistically significant(P0.05).The total effective rate 4 days after surgery was 30.0% in the experimental group,27.5% in the control group;the total effective rate 7 days after surgery was 90.0% in the experimental group,82.5% in the control group,the differences were not statistically significant.CONCLUSION The application of mannatide injection before the gallbladder cancer surgery can effectively improve the immunities of the patients and reduce the infection rate,which may not affect the clinical efficacy.
