Estimate the clinical evaluation of cystain C and some other indicator signal for monitoring the damage of renal function

Abstract

Objective Evaluating the cystain C(CysC) and urinary microalbumin(albumin,mAlb) to monitor the damage of renal function.Methods Testing the level of serum CystainC(CysC) in 74 patients who suffered from the type 2 diabetes,with latex particles enhancing immune transmission turbidimetry,and meanwhile we tested their serum creatinine(Scr),blood urea nitrogen(Bun) and related metabolic markers.We gleaned the 24-hour urine specimens,calculated the creatinine clearance rate(Ccr) and tested urinary microalbumin.According to the level of Ccr,the diabetic patients were divided into four groups,namely A(Ccr 120 mL/min),B(120 mL/min≤Ccr 70 mL/min),C(70 mL/min≤Ccr≤50 mL/min)and D group(Ccr 50 mL/min).Comparing with the diagnostic sensitivity of CysC,mAlb,Scr,Bun between groups and made a correlation analysis.Results The type 2 Diabetic kidney damage initially expressed high Glomerular filtration,the Ccr was higher than normal level,in this period,comparing with control,CysC and Scr had a significant statistic difference(P = 0.048,P = 0.025);when Ccr in normal range,only Bun had a significant difference comparing with control group(P = 0.009);When Ccr dropped below the normal range,every marker showed significant difference comparing with control group.The correlation analysis showed that CysC,mAlb,Scr,Bun was all associated with Ccr,moreover CysC had the strongest correlation with Ccr(r =-0.628).Conclusion In the different periods of diabetic kidney damage,every indicator of conventional renal function had different specificity and sensitivity.To evaluate the damage's degree,all the indicators should be considered and made a judgment according to clinical history.In this research,CysC was the strongest maker that could reflect the change among every period of diabetic kidney damage
