Objectives: To explore the effects of two different COH protocols on ovarian follicular concentration of IL-12,IL-15,IL-18 and IL-12 and 23 subunit p40 in PCOS patients who received IVF-ET surgery,and to explore the best COH plan for patients.Methods: Sixty PCOS patients who received IVF-ET surgery between March 2008 and June 2009 in our hospital were divided randomly into two groups: 30 patients(Group A) were subjected to a long protocol of gonadotropin-releasing hormone agonist(GnRH),30 patients with PCOS(Group B) were subjected to a protocol of GnRH antagonist.Then we compared the two groups in terms of the concentration of blood serum hormone levels on the days of hCG injecting,eggs removal,and embryo transfer.The ovarian follicular concentration of IL-12,IL-15,IL-18 and p40 subunit of IL-12 and 23 were also compared in the two groups.Results: The level of serum estrogen(E2) on the HCG injecting day and ovarian follicular concentration of IL-15 and IL-18 of group B patients were significantly lower than those of group A(P0.05).Group B were significantly lower than Group A in the number of Gn days,dosage of Gn and the number of eggs(P0.05).There were no significant difference between the two groups in serum levels of E2、P on days of egg removal and embryo transfer.No difference was found in the level of E2、P、p40 subunit of IL-12 and IL-23 in the ovarian follicular,the fertility rate,pregnancy rate,and abortion rate(P0.05).The rate of moderate OHSS in group B was less than that in group A,but with no statistical significance(P0.05).Conclusion: For PCOS patients undergoing IVF-ET,GnRH antagonist administration can not only have a good clinical pregnancy outcomes,but also decrease the incidence of the OHSS.
