Noninvasive ventilation and invasive ventilation in patients with severe type IIrespiratory failure:a randomized controlled trial

Abstract

Obstract:Objective To estimate the efficacy of noninvasive ventilation in the patients with severe exacerbation of COPD.Methods Forty-six patients with severe COPD were divided into two groups at random,invasive ventilation group(23 cases) were treated with invasive mechanical ventilation and routine drugs;noninvasive ventilation group(23 cases) were treated with non-invasive mechanical ventilation and routine drugs.Results After 2h in invasive ventilation group,PaO2 was higher than that of pre-treatment,PaCO2 was lower(P0.05).The mean time of invasive mechanical ventilation was(8±4)d,the success rate was 91.7%;after 2h in noninvasive ventilation group,PaO2 was higher than that of pre-treatment(P0.05),PaCO2 dropped inapparently(P0.05),8 patients changed invasive ventilation,the reduction rate of intubation was 60.9%.The mortality was 26.1% in noninvasive ventilation group,it was higher than that in invasive ventilation group(P0.05).Conclusion The effects in improvement of PaO2 and PaCO2 in invasive ventilation group was better than that in noninvasive ventilation group,the mortality in noninvasive ventilation group was higher than that in invasive ventilation group.
