The application of noninvasive ventilation in chronic obstructive pulmonary disease and respiratory failure

Abstract

Objective To discuss the curative effect of chronic obstructive pulmonary disease(COPD) and type Ⅱ respiratory failure patients with the use of noninvasive ventilation.Methods COPD patients with respiratory failure in 65 cases were divided into noninvasive ventilation group and invasive ventilation group,before and after treatment in 6 hours,48 hours of arterial blood gas analysis were observed:arterial partial pressure of oxygen(PaO2),partial pressure of carbon dioxide(PaCO2),pH,and heart rate(HR) changes.Results Noninvasive ventilation treatment after 6 hours,the PaO2 increased,PaCO2 decreased(P 0.05),pH value of recovery,decreased heart rate was significantly lower than that in invasive ventilation group,but after 48 hours the PaO2,PaCO2,pH,HR were not significant difference.Noninvasive ventilation for patients on time and length of hospital stay was significantly less than that invasive ventilation group(P 0.01),complications of nosocomial invasive ventilation group 18 cases,accounting for 54.5%;noninvasive ventilation group 6 patients,nosocomial infection rate was 18.8%(P 0.01).Noninvasive ventilation group had 2 cases ineffective treatment,the tracheal intubation after the invasive ventilation,the rest patients all by noninvasive ventilation treatment bebber and released.Conclusion Chronic obstructive pulmonary disease with type Ⅱ respiratory failure with noninvasive ventilation therapy is a safe and effective method.
