Abstract
AIM To study the effect of dexmedetomidine on stress reaction in difficult airway patient with pass nasus intubation.METHODS Forty ASAⅡ-Ⅲpatients with anticipated difficult airways were randomly divided into 2 groups(20 in each).In group D,dexmedetomidine 1μg·kg~(-1) was administrated for 10 minutes,and sufentanil 0.6μg·L~(-1) by target controlled infusion in group S.The blood pressure(BP),pulse oxygen saturation (SpO_2) and heart rate(HR) were recorded before anesthesia(T_0),fibreoptic pass later nostril(T_1),epiglottis (T_2),intubation succeed(T_3),1 min after intubation(T4) and 3 min after intubation(T_5).Plasma norepinephrine(NE),epinephrine(E),Cortisol(Cor) and blood glucose(BG) were detected.Hoarseness, sore throat and awareness of the intubation were also recorded 24 h after operation.RESULTS There were no significantly differences in BP between the two groups(P 0.05).HR at T_-T_4 in the group S were significantly increased compared with that at T_0(P 0.05),and decreased markedly compared with that at T_5 in the group D(P 0.05).HR at T_1-T_5 in the group D were significantly decreased compared with those at the same point in the group S(P 0.05).SpO_2 at T_1 and T_2 in the group S were significantly lower than T_0(P 0.05),and lower than group D at the same point(P 0.05).The levels of NE,E,Cor and BG showed no significant changes in the group D(P 0.05).Cor at T_3 in the group S was higher than that at T_0 and T_5,and significantly higher than that at T_3 in the group D(P 0.05).The occurrence rates of hoarseness and sore throat showed no significant difference between two groups(P 0.05),while awareness of the intubation were lower in the group D(P 0.05).CONCLUSION For difficult airway patient with underewent nasus intubation,stress response is inhibited and awareness of the intubation is depressed during dexmedetomidine infusion.
