Clinic Observation of Compound Trabeculectomy for Angle-closure Glaucoma

Objective To evaluate the clinical effect of combined surgery of classic trabeculectomy with adjustable suture and amniotic membrane implantation(Compound trabeculectomy) on patients with angle-closure glaucoma.Methods This study comprised ninety-three patients(128 eyes) that had angle-closure glaucoma with 1/2-3/4 quadrant angle being closed due to different causes admitted into our hospital between January 2010 and January 2011.Forty-eight patients(64 eyes) and forty-five patients(64 eyes) were randomly included in the observation group treated with the compound trabeculectomy and in the control group treated with classic trabeculectomy alone,respectively.Postoperatively,the intraocular pressure(IOP),filtering bleb morphology,visual acuity and complications were recorded.The follow-up time was at least 12 months after surgery.Results IOP of 58 eyes,5 eyes and 1 eye achieved complete success,conditional success and failure in the observation group,respectively,and the total success rate was 98.4%.IOP of 42 eyes,6 eyes and 16 eyes achieved complete success,conditional success and failure in the control group,respectively,and the total success rate was 75%.The difference of IOP between groups was statistically significant(χ2=15.89,P0.05).48 eyes,9 eyes,6 eyes and 1 eye formed typeⅠ,Ⅱ,Ⅲ and Ⅳ filtering bleb in the observation group,respectively,and the formation rate of functional filtering bleb was 89.1%.29 eyes,10 eyes,19 eyes and 6 eyes formed typeⅠ,Ⅱ,Ⅲ and Ⅳ filtering bleb in the control group,respectively,and the formation rate of functional filtering bleb was 60.9%.The difference between groups was statistically significant(χ2=15.07,P0.05).The rate of grade 0 visual acuity in the observation group and in the control group was 56.3% and 34.4%,respectively,and there was statistical difference between groups(χ2=6.18,P0.05).There were less complications in the observation group.Conclusion Compound trabeculectomy might effectively increase success rate of surgery for angle-closure glaucoma,with less complications.
