An evaluation of surgical treatment of V pattern exotropia

Objective To investigate the clinical features of V pattern exotropia and to evaluate the effects of surgical treatment of V pattern exotropia.Methods 43 cases of exotropia with V pattern were studied.The inferior oblique muscle weakening procedure combining the common horizontal muscle operation was performed on 38 cases,and 5 cases were treated with the horizontal rectus vertical transposition.Eye positions,the function of the inferior oblique muscle and the binocular vision before and after operation were analyzed.Results The weakening procedure of inferior oblique muscle combining the common horizontal rectus muscle operation was the most effective in most V pattern exotropia.The horizontal rectus vertical transpositon was effective for V pattern exotropia without apparent hyperfunction of inferior oblique muscle.After the operation,eye position of 37 patients was satisfactory.Hyperfunction of inferior oblique muscle disappeared in 54 eyes,weakened in 8 eyes.15 patients got partly the function of the binocular vision.Conclusion Different surgical procedures should be selected according to hyperfunction degree of inferior oblique muscle,meanwhile the horizontal strabismus should be treated by common horizontal rectus recession-resection procedure.After operation,we got good results.The function of the binocular vision was possibly improved.
