Surveillance on drug-resistance of extended-spectrum β-lactamases-producing Klebsiella pneumonia from old patients

Objective To investigate drug-resistance of extended-spectrum β-lactamases(ESBLs)-producing Klebsiella pneumonia isolated from old patients from August 2001 to June 2004.Methods Two hundred and ninety-four strains of Klebsiella pneumonia isolated from patients60 years old were identified and performed antimicrobial susceptibility tests by Micro Scan A/S-4 system,ESBLs were detected with disc confirmatory test,antimicrobial resistant rates of ESBLs positive and negative strains were analysed statistically.Results Two hundred and three of 294 Klebsiella pneumonia strains produced ESBLs,the positive rate was 69.05%;75.51% strains were resistant to more than 3 kinds of antimicrobial agents,84.23% of which were ESBLs positive strains.In addition to imipenem,the resistant rates of ESBLs positive strains to 20 kinds of antimicrobial agents were significantly higher than those of ESBLs negative strains(P0.005～0.025).ESBLs-producing strains had the lowest resistant rate to imipenem,which was 2.96%;followed by cefepime(18.23%),cefoperazone/sulbactam(22.66%),ceftazidime(27.59%) and cefoxitin(28.08%).Conclusion The isolation rate of ESBLs-producing Klebsiella pneumonia from old patients was high,drug-resistance and multidrug-resistance was common.Prevention,monitor and treatment of infection caused by ESBLs -producing strains should be paid attention.
