Percutaneous nephrolithotripsy with pneumatic and ultrasonic power under B ultrasound guidance in standard access for treatment of complex renal calculi

Abstract

Objective To evaluate the safety and efficacy of combined pneumatic and ultrasound lithotripsy in percutaneous nephrolithotomy(PCNL) for complex renal calculi under B ultrasound guide.Methods From December 2006 to January 2009,123 patients(128 kidneys) were treated by combined pneumatic and ultrasound lithotripsy in PCNL,with the standard percutaneous renal access established by B ultrasound guide.Results The standard percutaneous renal access was successfully established in all patients.3 patients had two accesses established respectively in one kidney during the first phase,1 case received delayed phraseⅡlithotripsy.The rate of stone clearance was 90.63%.Residual stone fragment was found in 3 patients who received second PCNL one week later,6 patients had residual stones smaller than 1cm of diameter and received treatment of ESWL after operation.3 patients with residual micro-stones were informed to examine regularly after leaving hospital.6 patients developed bacteremia and 1 retroperitoneal hematoma after operation.No surgical complications occurred during lithotripsy during the 1 to 24 months of follow-up.Conclusion It is effective and safe to treat the lithotripsy of combined pneumatic and ultrasound lithotripsy guide by B ultrasound in percutaneous nephrolithotomy in complex renal calculi.
