The Long-term Follow-up Study of 683 Cases Glottic Carcinoma

Objective To study the long-term follow-up result of glottic carcinoma and reservation of laryngeal function.Methods six hundred and eighty-three patients with glottic carcinoma from 1986 to 2002 were summarized(T1 256 cases,T2 187,T3 189,T4 51,accord-ing to UICC in 2002).In common 7 kinds of operations were performed:92 cases underwent cordectomy,243 vertical laryngectomy,7 frontolateral laryngectomy,12 horizontal glottic laryngectomy(middle part of the larynx),106 subtotal laryngectomy with cricoglossoepiglottic anastomosis,86 laser laryngectomy,137 total laryngectomy.197 cases underwent concurrent neck dissection(31 bilateral,166 unilateral).Results The overall three,five and ten year survival rates of the glottic carcinoma were 91.8%,85.7% and 72.3% respectively.All patients with partial laryngectomy restored their phonation and overcame aspiration with removing nasal feeding from 7 to 25 days after operations.451 cases were decannulated from 9 days to 3 months after operations.Decannulation rate was 98.9%.Conclusion Mastering indications strictly,correct operation choices,excellent surgical skills and perfect repairing technique are bases of improving life qualities and curative effect.
