Endovascular Management of Carotid Cavernous Fistula

Abstract

Introduction: Carotid cavernous fistula is a rare neurovascular pathology, it represents an abnormal communication between the cavernous segment of the internal carotid artery and the cavernous sinus, and its etiology either traumatic or spontaneous has a drastic impact on the patient's life. Method(s): Different methods were described for the endovascular management of CCF, either detachable balloons, coils, or liquid embolic agents. We describe our experience in management of CCF using different techniques either transarterial or transvenous route with and without balloon protection. Result(s): A total of 11 patients with CCF were included in our study where 9 patients were direct CCF and 2 patients were indirect type, 8 patients were treated through direct coiling through an arterial route with and without balloon assistance, 2 patients through the IPS and 1 patient through ECA branches. Conclusion(s): CCF is a rare neurovascular pathology where the most common etiology in Egypt is traumatic and the endovascular treatment of CCF is feasible, safe, and effective curative treatment.
