Abstract
A 34-year-old woman presented with severe visual loss on the left eye secondary to a large anterior clinoid process meningioma with internal carotid artery (ICA) encasement. The patient underwent an orbito-zygomatic craniotomy with extradural anterior clinoidectomy and early optic canal decompression. Clinoidal ICA was identified first to safely expose supraclinoidal ICA after distal dural ring transection. Gross total resection was obtained with dramatic improvement of visual acuity on the left side.
