Abstract

This chapter provides the basic characteristics, side effects/toxicity, drug interactions, and dosage information of the Tyzeka (Telbivudine). Telbivudine is a synthetic thymidine nucleoside analog with activity against hepatitis B virus (HBV). It is phosphorylated by cellular kinases to the active triphosphate form that inhibits HBV DNA polymerase (reverse transcriptase) by competing with the natural substrate, thymidine 5'-triphosphate. Incorporation of telbivudine 5'-triphosphate into viral DNA causes DNA chain termination. Telbivudine is indicated for the treatment of chronic hepatitis B in adult patients (16 years or older) with evidence of viral replication and either persistent elevations in serum aminotransferases or histologically active disease. Telbivudine can be taken with or without food. Telbivudine should not be given with pegylated interferon alfa-2a. Because telbivudine is eliminated primarily by renal excretion, coadministration of telbivudine with drugs that alter renal function may alter plasma concentrations of telbivudine.
