Laboratory Profile for Different Types of Hemolytic Anemia in Children

В статье представлены результаты оценки лабораторных показателей при различных вариантах гемолитической анемии (ГА) у детей. Всего обследовано 69 пациентов в возрасте от 1 месяца до 18 лет. Преобладали наследственные варианты ГА (71%), среди которых лидировал наследственный сфероцитоз (НС) - 64% (n=37); талассемия - у 18,4%; ферментопатии - 6,1%. Анализировались лабораторные показатели общего анализа крови, включая эритроцитарные индексы: MCV (mean corpuscular volume) - средний объем эритроцита; МСН (mean corpuscular hemoglobin) - среднее содержание гемоглобина в эритроците, и биохимические показатели гемолиза (общий билирубин и фракции, лактатдегидрогеназа (ЛДГ), гаптоглобин). Дополнительно определяли показатели обмена железа и осмотическую резистентность эритроцитов (ОРЭ). При инициальной диагностике талассемия сопровождалась достоверно более низкими показателями эритроцитарных индексов: MCV (fl) - 56,3 (53,5; 57,7) и MCH (pg) - 19,3 (19,2; 19,8). При НС выявлена тенденция к микроцитозу при нормальных значениях MCH. Так, показатель медианы MCV при НС составил 77,8 (72,9; 81,7) fl. Лабораторный профиль по данным биохимического анализа крови характеризовался умеренной гипербилирубинемией, повышением ЛДГ, снижением уровня гаптоглобина и нормоферемией при всех вариантах ГА. Однако при ферментопатиях отмечено более выраженное повышение уровня ЛДГ - 407 (336; 736) Ед/л и сывороточного ферритина - 246 (220; 256) мг/л при сравнении с другими ГА (p<0,0001). При НС наблюдалось выраженное снижение минимальной ОРЭ в инкубированной крови 0,75 (0,75; 0,8) %. Ферментопатии характеризовались повышением максимальной ОРЭ после инкубации: от 0,3 (0,3; 0,4) % до 0,25 (0,25; 0,45) % (р<0,0001). При аутоиммунной ГА выявлено снижение максимальной ОРЭ после инкубации: от 0,26 (0,25; 0,35) % до 0,35 (0,27; 0,4) %. При талассемии изменений ОРЭ не наблюдалось. Таким образом, при первичной диагностике ГА важно учитывать показатели эритроцитарных индексов. При этом определение ОРЭ до инкубации и после инкубации образца крови остается базовым методом лабораторной диагностики НС и может быть информативно при дифференциальной диагностике ферментопатий и аутоиммунной ГА. The article presents the results of assessement of laboratory parameters in various types of hemolytic anemia (HA) in children. In total we have examined 69 patients from 1 to 18 months old. Hereditary variants of HA prevailed (71%), among which hereditary spherocytosis (HS) was more common - 64% (n=37); thalassemia - in 18.4%; fermentopathy - 6.1%. The laboratory parameters of the general blood test were analyzed, including erythrocyte indices: MCV (mean corpuscular volume) - the average volume of an erythrocyte; MCH (mean corpuscular hemoglobin) - the average hemoglobin content in the erythrocyte and the biochemical parameters of hemolysis (total bilirubin and fractions, lactate dehydrogenase (LDH), haptoglobin. Additionally, the indicators of iron metabolism and osmotic resistance of erythrocytes (ORE) were determined. At the initial diagnosis, thalassemia was accompanied by significantly lower erythrocyte indices: MCV (fl) - 56.3 (53.5; 57.7) and MCH (pg) - 19.3 (19.2; 19.8). In HS, a tendency to microcytosis was revealed, with normal MCH values. Thus, the median MCV in HS was 77.8 (72.9; 81.7) fl. The laboratory profile according to the biochemical blood test was characterized by moderate hyperbilirunemia, increased LDH, a decrease in the level of haptoglobin, and normoferemia in all types of HA. However, in fermentopathies, a more pronounced increase in LDH level - 407 (336; 736) U/L and serum ferritin - 246 (220; 256) mg/L was noted when compared with other HA (p<0.0001). In HS, there was a pronounced decrease in the minimum ORE in the incubated blood 0.75 (0.75; 0.8)%. Fermentopathies were characterized by an increase in the maximum ORE after incubation: from 0.3 (0.3; 0.4)% up to 0,25 (0.25; 0.45)% (р<0.0001). In autoimmune HA, a decrease in the maximum ORE after incubation was revealed: from 0.26 (0.25; 0.35)% up to 0.35 (0.27; 0.4)%. In thalassemia, no changes in ORE were observed. Thus, it is important to take into account the parameters of erythrocyte indices in the primarydiagnosis of HA. At the same time, the ORE determination before and after incubation of a blood sample remains the basic method of laboratory diagnosis of HS and can be informative in the differential diagnosis of fermentopathies and autoimmune HA.
