Relationship between body mass index,waist circumstance and hypertension among community residents in Nanhai District,Foshan City

Abstract

Objective To comprehensively evaluate the risk for the occurrence of hypertension among adults with central obesity in Nanhai District,Foshan City,Guangdong Province by waist circumstance(WC) and body mass index(BMI).Methods Multistage stratified random sampling method was used to select permanent residents aged 18 years and above from 17,124 families in 2 streets and 6 towns of Nanhai District.The data of these residents were collected with questionnaires by face- toface interview.Physical examinations and blood pressure determination were taken by trained interviewers.Multi- factor Logistic regression analysis was conducted by SPSS13.0 statistical software.Odds ratio for hypertension associated with central obesity was compared among the 4 BMI groups.Results The prevalence rates of hypertension in the male subjects with central obesity of the lean,normal- weight,over- weight and obesity BMI groups were 14.6%,23.8%,34.1% and 42.7% respectively,and those in the female subjects with central obesity of the above- mentioned groups were 20.3%,24.2%,33.8% and43.5% respectively.Except for the male subjects of the lean BMI group,the prevalence rates of hypertension in the subjects with central obesity of the other 3 groups were significantly higher than those in the subjects with normal WC of the same BMI groups(P0.01).After adjusting the confounding factors(age,smoking,alcohol consumption,physical exercise and so on),odds ratios for hypertension in the male subjects with central obesity of the normal- weight,over- weight and obesity BMI groups were1.485(1.319-1.672),1.827(1.536-2.191) and 2.849(1.659-4.893)respectively when compared with the subjects with normal WC of die same BMI groups.Odds ratios for hypertension in the female subjects with central obesity of the lean,normal-weight,over-weight and obesity BMI groups were 1.642(1.112-2.425),1.530(1.363-1.717),1.644(1.299-2.080)and 3.529(2.001-6.225) respectively.Conclusions The specificity and sensitivity of diagnosing hypertension are significandy promoted by a combination of BMI and WC.The population with abdominal obesity,especially those with systemic obesity and abdominal obesity,should be treated as the key intervention subjects for prevention and control of hypertension in communities.
