Carcinoma ex pleomorphic adenoma is a rare neoplasm of the salivary gland. This lesion, also known as malignant mixed tumor, occurs when a malignant tumor arises in the epithelial component of a pleomorphic adenoma. In my case, histologic examination revealed that tumor was composed partly of benign pleomorphic adenoma and partly of an adenocarcinoma component without capsular invasion. I experienced the first case of noninvasive carcinoma ex pleomorphic adenoma in parapharyngeal space and report it with a review of the literature. (J Clinical Otolaryngol 2005;16:316-319)
