Abstract
Objective To report the therapeutic effect of prophylactic vitrectomy in acute retinal necrosis syndrome(Ⅰ~Ⅱ stage).Methods To analyze retrospectively therapeutic data of the prophylactic vitrectomy for acute retinal necrosis syndrome(Ⅰ~Ⅱ stage)from February,2006 to July,2008.All patients underwent total vitrectomy combined with photocoagulation and silicone oil temponade.Triamcinolone acetonide was used to make the vitreous body visible in the course of operation.Postoperatively,prone position was conducted regularly.All patients were treated with acyclovir preoperatively and postoperatively,followed up for 10~12 months.Results The retina attached in 18 eyes atter extraction of silicone.The retina detached in two eyes,while the retina of one eye attached after the second peeling membrane,tamponade of silicone oil and photocoagulation.The retina attached after silicone oil was extracted.While the other eye underwent the second surgery,the retina did not attach because the retina necrotized and the retinal arteries occluded vastly before operation.Visual acuity increased in 13 eyes,no changed in 5 eyes,decreased in 2 eyes.Conclusions The prophylactic vitrectomy combined with photocoagulation and silicone temponade is effective for the acute retinal necrosis syndrome.Visual acuity can be improved or kept after operation.The incidence was decreased in retinal detachment of ARNS.
