The comparative analysis of the formation of functional filtration bleb in different incision of scleral flap after trabeculectomy

Objective To assess the success rate of the formation of functional filtration bleb in eyes undergoing different incision of scleral flap in glaucoma trabeculectomy.Methods The formation of functional filtration bleb after trabeculectomy was studied.Two different mode scleral flaps in trabeculectomy were performed in 339 cases (339 eyes) of primary glaucoma.The small scleral flap (2mmx2mm) was 149 eyes and the big scleral flap (4mmx4mm) was 190 eyes.Results All patients were observed after trabeculectomy in one week,but only 117 eyes were followed up in three months.In the small scleral flap,the success rate of the formation of the filtration bleb was 82.55％ after trabeculectomy in one week,55.36％ in three months.In the big scleral flap,the success rate of the formation of the filtration bleb was 67.89％ after trabeculectomy in one week,77.05％ in three months.There was significant difference between the two groups in terms of the success rate of the formation of the filtration bleb.Conclusions The clinical observation showed that the long-term effect was especially effective and safer for the big scleral flap.The trabeculectomy with big scleral flap should be performed for primary glaucoma to improve the outcome.
