Impacts of transitional nursing intervention in patients with type 2 diabetes

Abstract

Objective To explore the impacts of transitional nursing intervention in patients with type 2 diabetes. Methods A total of 80 patients, who treated for type 2 diabetes from the period between January 2013 and January 2014, were selected by convenient sampling method and were randomly divided into observation and control group, with each group 40 cases. The patients of control group received the routine nursing care, while the patients of observation group received additional transitional nursing intervention based on the routine nursing care. The level of understanding relative knowledge, the hospitalization rate after discharge for one year, mortality rate and the degree of satisfaction to nursing care were compared between before and after interventions. Results There were two deaths in observation group and four deaths in control group one years after patients discharging. The questionnaire score in relative knowledge of diabetes were not statistically different between two groups before the intervention (P>0.05); the score of observation group has increased and was statistical significant higher than control group after the transitional nursing intervention (P<0.05). The hospitalization rate of observation group was 22.5% and the mortality rate was 5% after one-year follow up, while those of control group were 37.5% and 10.0% (χ2=3.916, 5.024; P=0.047, 0.025). The average hospitalization was (8.21±4.32)d in observation group and (8.21±4.32)d in control group (t=2.237, P=0.022). After the follow-up, the degree of satisfaction to nursing care was 100% in observation group and 73.5% in control group (P<0.05). Conclusions The transitional nursing intervention can enhance the relative knowledge about disease in patients with type 2 diabetes, reduce the hospitalization rate and duration of hospitalization, and improve the degree of satisfaction to nursing care among patients.
