Fibroblast growth factor-23 (FGF23) is a phosphaturic hormone, and its levels increases as kidney function declines. Increased levels of FGF23 occur primarily as a result of a net positive phosphate balance leading to increased renal phosphate excretion. Although FGF23 helps to prevent hyperphosphatemia, elevated circulating levels are independently associated with vascular dysfunction, left ventricular hypertrophy, and increased risk for death in patients with CKD.
