Abstract

Background: No data are available regarding the observed mortality after MitraClip in patients with symptomatic heart failure (HF) and significant functional mitral regurgitation (FMR) compared to the mortality predicted by the available HF scores. Methods and results: From 2008–2015, 171 patients with moderate-to-severe HF received MitraClip at two institutions. Patients were stratified according to aetiology in non-ischemic FMR (n=64) and ischemic FMR (n=107). The observed mortality was compared with that predicted by the Seattle Heart Failure Model (SHFM), the heart failure calculator of the meta-analysis global group in chronic heart failure (MAGGIC) and the Cardiac and Comorbid Conditions HF (3C-HF) Score after 1-year: 20% observed, 20% by SHFM (Δ=0), 26% by MAGGIC (Δ=-6%) and 45% by 3C-HF (Δ=-25%). At 2-year: 32% observed vs. 34% by SHFM (Δ=-2%). At 3-year: 44% observed vs. 52% by MAGGIC (Δ=-8%). At 5-year: 63% observed vs. 60% by SHFM (Δ=+3%). Patients with non-ischemic FMR had the lowest mortality after 1-year (12%) compared to that predicted by SHFM (17.5%, Δ=-5.5%), MAGGIC (24%, Δ=-12%) and 3C-HF (41%, Δ=-29%).
