LBPS 02-09 DETERMINES OF UNCONTROLLED MORNING BLOOD PRESSURE MEASURED BY MEMORY-EQUIPPED DEVICE AMONG ELDERLY HYPERTENSION PATIENTS IN PRIMARY CARE CLINICS

Abstract

Objective: Increased morning blood pressure has been associated with fatal and nonfatal cardiovascular events, especially in Asians. However, the prevalence and determines of uncontrolled morning hypertension are little studied in Chinese population. Design and Method: We therefore included 707 elderly hypertensive patients in Xinzhuang County Hospital in Shanghai. The home morning blood pressures were measured three times by memory-equipped device for consecutive seven days. Results: The prevalence of uncontrolled morning hypertension is 51.3%. The uncontrolled morning hypertension was associated with age (OR: 1.074; 95% CI: 1.041-1.108), office systolic blood pressure (OR: 1.027; 95% CI: 1.015-1.039), office diastolic blood pressure (OR: 1.042; 95% CI: 1.021-1.064), the use of long acting antihypertensive drugs (OR: 0.588; 95% CI: 0.355-0.973) and the number of antihypertensive drug (OR: 1.387; 95% CI: 1.059-1.817). Comparing with office blood pressure, the home morning blood pressure showed more significant association with age and long acting antihypertensive drugs. Conclusions: In conclusion, the prevalence of uncontrolled morning hypertension is high in the elderly Chinese hypertension patients, and the home blood pressure monitoring might be a feasible method to detect it. To use long acting antihypertensive drugs and to perform enhanced office blood pressure reduction might be helpful to improve morning hypertension.
