高位腰椎椎間板ヘルニアの外科的治療は, ヘルニアの突出方向に対応してlateral typeに対してはUPH, mediolateral typeにおいても基本的にはUPH, ただしhuge discの場合はBPHあるいはTLが必要と考えられた.far-lateral typeにおいてはLA, medial typeに対してはTLあるいはTDAによりヘルニア摘出が可能と考えられた.
