More recent knowledge on pathogenesis and immunopathology of HIV-infection permit more hope for progress in prevention and treatment. The important steps of viral replication and persistence are restated. The considerable magnitude of HIV plasma viremia during all disease stages implies relevant conclusions concerning diagnosis, therapy and virulence. Variable pathogenesis of HIV-infection is based on viral features, cofactors and immunopathological events.
