Abstract
Objective: To probe into the applicant value of standard large trauma craniotomy in the treatment of severe brain injury.Methods: Selecting 79 cases of traumatic craniotomy by the neurosurgery department between May 2002 and May 2006,dividing them into two groups-one group with standard large trauma craniotomy(n=40) and the other with regular temporal valve and comparing the curative effect of these two groups.Results: According to the prognosis outcome six months after the injury,for standard major bone valve craniotomy there were 13 cases of good recovery,15 cases of severe disability and vegetables,12 cases of death;For regular trauma craniotomy,there were 5 cases of good recovery,severe disability and vegetables and 22 cases of death.Through comparison,significant difference has been found(P0.05).For the large trauma craniotomy group,intracranial decompression is better than in the regular bone valve group(P0.01), for standard large trauma craniotomy group,the patients had a notably lower rate of delayed hematoma,incision hernia and CSF leak than in the regular trauma craniotomy,but there is no significant difference regarding the rate of post-operation traumatic epilepsy,intracranial infection and traumatic hydrocephalus between these two groups.Conclusion: The large trauma craniotomy has a better curative effect and fewer complications than the regular trauma craniotomy.
