Correlation analysis of morning blood pressure surge in patients with hypertension.

Abstract

Objective To analyze the prevalence and the related factors of morning blood pressure surge(MBPS) in patients with hypertension.Methods Blood pressure of 415 patients with hypertension were subjected to 24 h ambulatory blood pressure monitoring(ABPM).The patients were then classified as MBPS(n=129) and non-MBPS(n=286)group.Patient information,such as the sex,age,ABPM,Holter recording,color doppler echocardiography,carotid artery intimal-medial thickness(IMT),blood lipid,fasting blood glucose(FBG) and body mass index were then compared.Results ①31.1% of the patients had MBPS.24 h mean systolic blood pressure and mean pulse pressure in MBPS group were significantly higher than the non-MBPS group(P0.001).②MBPS was significantly correlated with age(P0.01).After age adjustment,MBPS of the male patients was significantly higher than the female patients(28.5% vs 16.9%,P=0.0105).③Left ventricular hypertrophy(LVH),left ventricular mass index(LVMI),carotid artery IMT,24 h mean heart rate,atrial premature beat,ventricular premature beat of both male and female aptients in the MBPS group were significantly higher than the non-MBPS group(P0.001).Association between MBPS and BMI,FBG and blood lipid was not observed.Conclusion Patients with MBPS were found to have more severe carotid atherosclerosis,LVH and arrhythmias than the non-MBPS patients.Age and sex were the major factors associated with MBPS.MBPS may increase the risk of morning cardiovascular events.
