Clinical research on adult kidney transplantation with unilateral kidney from child donation after cardiac death

Objective To summarize operative experience of adult kidney transplantation with unilateral kidney from child donation after cardiac death(DCD) and to investigate the clinical efficacy and safety.Methods A five years and eight months old child donated two DCD kidneys,which were used in 2 adult recipients for kidney transplantation.The intraoperative and postoperative management experience was summarized.The two recipients were followed up for 1 year.The renal graft function,the change of graft kidney size and the occurrence of complications were monitored.Results The sizes of 2 graft kidneys were 8.0 cm×3.0 cm×4.0 cm(right kidney) and 8.2 cm×3.1 cm×4.3 cm(left kidney) respectively.Two recipients were both female with the weight less than 50 kg.The unilateral kidney was transplanted in right fossa iliaca.The operative method and immunosuppression protocols were the same as that in adult donor kidney transplantation.The serum creatinine(Scr) in two recipients decreased to normal level respectively at 10th days and 30th days after transplantation.The estimated glomerular filtration rate(eGFR) increased to stable level(50 ml/min) respectively at 15th days and 50th days after transplantation.The size of renal grafts grew to stable level and was close to the adult's size in two weeks.Two recipients were followed up until the manuscript was delived with normal renal function and urinary volume,without albuminuria and complications.Conclusions Adult kidney transplantation with unilateral kidney from child DCD(about 5 years old) is effective and safe in appropriate recipient.
