Abstract
Objective:To investigate the clinical and pathological characteristics of the idiopathic membranous nephropathy and analyze the prognostic factors. Methods:Retrospectively study and analyze the clinical data of 65 idiopathic membranous nephropathy cases. Conclusion:The clinical and pathological characteristics:patients were mainly elderly people. Male patients were of a higher proportion. Patients aged 60 and above were of a lower level of renal function. The vast majority of patients (81.5%) manifest nephrotic syndrome. About one-third of the patients were at CKD3 and above at the time of biopsy. Patients were mainly inⅠand Ⅱ pathological stages(80%). The majority of patients presented tubulointerstitial injuries. Kidney lesions tend to be in the active phases. Prognostic factor analysis showed that:In this study, the median of urinary protein is 2.718g/d. Patients with a 24-hour proteinuria more than 4g/d is less than expected. In this study,the median of urinary β2-MG valued 191.60μg/L is significantly higher than normal. Correlation analysis of the tubulointerstitial pathology integral with eGFR indicates that with more severe the degree of tubulointerstitial injury in patients,the lower renal function is. Taking pathological stage for the criterion,analyze the distribution of the eGFR,serum ALB and Upro pathology in different stages with Kruskal-Wallis H test. The results showed that the distributions of the three indicators in different pathological stages are of no significant difference(P0.05),prompting there may not be obvious links between pathological stages and prognosis.
