Effect of Propofol combined with Remifentanil in one-lung ventilation anesthesia on patients

Abstract

Objective:To observe the effects of Propofol combined with Remifentanil anesthesia on the respiration,circulation system and stress reaction of patients during one-lung ventilation(OLV).Methods:60 cases of ASAⅠ-Ⅱ patients for elective thoracic surgery were randomly divided into observation group and control group,each of 30 cases.Total intravenous anesthesia was given to the observation group(group Ⅰ),which was maintained with Propofol 1-3 μg/ml and continuous Remifentanil 0.10-0.25 μg/(kg·min),intermittent intravenous injection Vecuronium 0.02 mg/kg.Intravenous-inhalation anesthesia was given to the control group(groupⅡ),which was maintained with 1%-3% Isoflurane inhalation and continuously intravenous Remifentanil 0.10-0.25 μg/(kg·min),intermittent intravenous injection Vecuronium 0.02 mg/kg.BP,MAP,HR,SpO2,ECG,CSI and PetCO2 were continuously monitored.Blood samples were taken from the radial artery before anesthesia(T0),5 min before skin incision(T1),5 min after skin incision(T2),30 min after skin incision(OLV,T3) and 10 min before the end of operation(T4) for measurement of blood gases and stress reaction index.Results:At T3,SpO2 in groupⅠ was significantly higher than that of group Ⅱ(P0.05);at T1,T2,T4,PaO2 in groupⅠ was significantly higher than that of group Ⅱ(P0.01);at T1,SBP in groupⅠ was significantly lower than that of group Ⅱ(P0.01);at T1,T4,HR in groupⅠwas significantly lower than that of group Ⅱ(P0.05);at T1,DBP in group Ⅰ was significantly higher than that of group Ⅱ(P0.05);at T4,DBP in groupⅠ was significantly lower than that of group Ⅱ(P0.05);at T3,T4,Cor in groupⅠwas significantly lower than that of group Ⅱ(P0.01,P0.05);at T4,Angiotensin Ⅱ in group Ⅰ was significantly higher than that of group Ⅱ(P0.05);at T1,T2,T3,T4,IL-6 in group Ⅰ was significantly lower than that of group Ⅱ(P0.01);INS in each time of the two groups,the difference was not statistically significant(P0.05).Conclusion:Compared with Isoflurane,Propofol combined with Remifentanil anesthesia during OLV can stabilize the respiratory and circulatory system,reduce the stress response,and there are few complications during and after operation.
