The Efficacy and Safety of Irbesartan Treatment in Renal Function in Hypertensive Patients

Abstract
Objective To investigate the renal protective effects of irbesartan in patients with essential hypertension(EH) . Methods Senty-six patients with EH complicated with mild insufficiency of renal function and 38 normotensive subjects were recruited. Fifty cases were treated by irbesartan with 150-300 mg/d and 26 cases by enalapril 5-10 mg/d for 12 months. The alteration of renal hemodynamics and structure, blood and urine microglobulin of β_~2 -MG before and after treatment were determined. Results Compared to the normontensive subjects, β_~2 -MG in blood and urine were obviously increased. RI, PI and Vm were markedly increased before treatment(P0.01). Systolic and diastolic pressure were significantly decreased(P0.01) in group irbesartan, β_~2 -MG in blood was decreased from (3.7±0.7 to 2.4± 0.3)mg/L (P0.01); urine β_~2 MG was decreased from (561±71)μg/L to (148±16)μg/L(P0.01). RI, PI and Vm were markedly decreased in kidney after treatment. Thickness of renal arterial wall was significantly decreased from (3.31±0.32 to 2.41±0.52)mm in left kidney, the renal arterial diameter were obviously increased from (4.25±0.72 to 4.86±0.71)mm (P0.05). Compared to enalapril group, irbesartan markedly decreased RI, PI and Vm in left and right kidney. Conclusion Irbesartan was effective in reducing hypertension along with the concentration of β_~2 -MG in blood and urine, renal arterial vessel resistance.
