Surgical induced eruption and orthodontic treatment for the impacted curved upper teeth

PURPOSE:To study the effect of treating the impacted curved upper teeth which couldn't erupt by orthodontic traction.METHODS:24 cases with 26 impacted curved upper teeth were treated with surgical exposure and orthodontic traction after X-ray location and tooth space expansion.RESULTS:In 24 cases,seventeen teeth(65.4%) was successfully treated.six teeth(23.1%) was approximately erupted.3 teeth(11.5%) failed.The average eruption time of 26 impacted teeth was 7.5 months.The longest time was 13 months.The shortest time was 3 months.All the teeth had vital pulp and no root absorption or conglutination was found.There was labial gingival regression with 2-4mm in 3 teeth.The loss of marginal bone was 1.0 mm and 2.5mm in 2 teeth respectively.In 1 case,the tooth couldn't be moved into alignment and the light-cured or porcelain crown treatment was needed.In 1 cases,the teeth couldn't be moved into alignment.In 2 cases,the tooth was extracted.All the other teeth moved into alignment.CONCLUSIONS:Strict selection of indication,exact location,enough space,appropriate traction force,anchorage and control of inflammation are the keys to success.
