Chinical Study of MR Diffusion Weighted in the Perihenatomal Edema of Acute Intracerebral Hemonhage

Abstract

Objective To investigate the characteristic of the perihematomal edema in acute intracerebral hemorrhage (ICH) and discuss the information mechanism by using MR diffusion weighted imaging.Methods 44 patients with supratentori- al ICH within 2 weeks after upsets were involved,and the edema was observed in all these cases.The apparent diffuse coefficient (ADC) values of the perihematomal edema area and conlaterial symmetric area were compared and the correlation analysis was performed between the relative ADC values and edema volume.Results Prihematomal edema was observed in all 44 patients. The ADC values increased in 40 cases,elevated 58.6% (P<0.01) in edema area 24.1% (P<0.001) around hematoma 1 cm compared with conlateral symmetric area.Relative ADC values were correlated with absolute edema volume and relative ede- ma volume (P=0.003).Significant ADC decrease was observed in the edema area of other 4 cases.Conclusion In acute stage,edema ADC values increase in most cases,and vasogenic edema is the major information mechanism.However,there is a subset of ICH patients with decreased ADC values,and cytotoxic edema predominated.Although it occurrs only in a minority of patients,it maybe linked with serious clinical status and worse outcome.
