Abstract
Objective To observe the therapeutic effect of core decompression and bone graft through trochanter on femoral head avascular necrosis based on the ARCO classfication.Methods 65 cases (80 hips) suffering from avascular necrosis of femoral head were treated with core decompression and bone graft through trochanter.According to the ARCO classification,80 hips were classified into stageⅠ(23 hips),stageⅡ(41 hips),stageⅢ(16 hips),and they were divided into group A(25 hips),group B(25 hips),group C(30 hips) according to the area of osteonecrosis.The outoome were evaluated on the function changes of hip joints and clinical and radio- graphic progression.Results The patients were followed up for 24~72 months,with an average of 40 months.The good and excellent rates ofⅠ,stageⅡ,stageⅢwere 82.61% ,65.85% ,31.25% respectively.There was significant difference among three stages(P= 0.0042).The good and excellent rates of group A,group B,group C were 84.00% ,64.00 % ,46.67 % respectively.There was signif- icant difference among three groups(P=0.0164).Conclusion The core decompression for femoral head osteonecrosis is rdated with its:the earlier,the better.It is also related with its area of osteonecrosis:the smaller,the better.
