Abstract
Objective To explore the operative application and therapeutic effect of the scleral cryotherapy under operation microscope on rhegmatogenous retinal detachment with peripheral and macular hole. Methods 12 cases(12 eyes) of rhegmatogenous retinal detachment with peripheral and macular hole were enrolled in this study. All cases underwent pars plana vitrectomy(PPV) combined with scleral buckling、scleral cryotherapy under operation microscope and intraocular gas(C3F8) tamponade.Postoperative patients were asked to posture face up or right or left side down for 10 days.Postoperatively, the effect on the reattachment rate and visual acuity were evaluated.Data were analysed statistically by Chi-Square test. Results Mean postoperative follow-up was 23.2 months(range 6~36 months),11 of 12cases achieved retinal reattachment after the initial surgery,1 of 12 cases achieved retinal reattachment after the twic surgery. The primary reattachment rate was 91.7%(11/12 cases). All peripheral and macular holes were finally closed. As compared with preoperative visual acuity and blindness rate, postoperative visual acuity was significantly increased and the blindness rate was significantly decreased(χ2=10.971,P=0.001).Conclusion PPV combined with scleral buckling、scleral cryotherapy under operation microscope and intraocular gas(C3F8) or silicon oil tamponade can achieve favorable anatomic outcomes in the treatment of rhegmatogenous retinal detachment with peripheral and macular hole.Scleral cryotherapy under operation microscope can shorten operative time.Moreove, intraoperative and postoperative complications are minimised. It is a safe and effective technique for the management of rhegmatogenous retinal detachment with peripheral and macular hole.
