Abstract
Objective To study the histologic mucosal changes and Helicobacter pylori(Hp) infection in children with primary bile reflux gastritis(BRG).Methods Fifty-five children with primary BRG and 106 children with chronic superficial gastritis children without bile reflux were selected;gastroscopic and histological mucosal changes were detected by gastroscope and histological method.The Hp infections in them were also determined by 13C-urea breast test(13C-UBT) and Hp-IgG.SPSS 10.0 software was used to analyze the data.Results In primary BRG group,the main histological mucosal changes were congestion,edema and inflammation cell invasion.The rate of Hp infection was 32.7% in BRG group,and 40.6% in the control group.The Hp infection rate and severity of gastric mucosa inflammation showed no significant difference between 2 groups.Frequencies of intestinal metaplasia,dysplasia and active inflammation were 9.1%,7.3%,5.5% in case group,and 1.9%,14.2%,10.4% in control group respectively.However,in the primary BRG children,the severity of gastric mucosa inflammation of Hp positive children was severer than Hp negative children,and there was significant difference between 2 groups(χ2=6.825 P0.05).However,the severity of gastric mucosa inflammation showed no significant difference between the BRG without Hp infection group and the chronic superficial gastritis without Hp infected group.Conclusions The gastroscopic and histological methods can be used for diagnosing BRG in children.There is no characteristic histological change in BRG of children.Bile reflux and Hp infection are the important factors causing gastric mucosal injury.Moreover,bile reflux often cause intestinal metaplasia in the gastric mucosa,but Hp infection often cause dysplasia and active inflammation.
