Clinicopathological study on 48 cases of patients with Henoch-Schonlein purpura nephritis

Abstract

Objective To analyze the feature of Henoch-Schonlein purpura nephritis(HSPN).Methods 48 cases of HSPN from Oct.2003 to Oct.2008 were divided into 4 groups:A hematuria and(or)24h urine protein1g/d,B hematuria and 24h urine protein1g/d,C combined with renal insufficiency and(or)hypertension,D nephrotic syndrome.The clinical data among groups were analyzed and compared.Results HSPN occupied 9.4% of total renal biopsy cases.65% were 11~30 years.Male:female 1.8∶1.Group A occupied 16.7%,type Ⅰ or Ⅱ in pathology;B 43.8%,type Ⅱ or Ⅲ;C 27.1%,type Ⅲ and above;D 12.5%,type Ⅱ or Ⅲ In C,there were 62.5% with tubule atrophy,42.9% with interstitial fibrosis,62.5% with inflammatory cell infiltration.87.5% with hypertension presented with thickening of the arteriole wall.The total effective rate is 87.5%,with better effect in mild patients.Leflunomide or mycophenolic acid had gained comparing result as cyclophosphamide.Conclusion HSPN was common in young adults.There were some extent of parallel relations between clinical presentations,pathological changes,and therapeutic effect.
