Microsurgical treatment for para-superior sagittal sinus meningiomas in middle cortex area

Objective To investigate the operative technique for the para-superior sagittal sinus meningiomas in the middle cortex areas and its curative effect.Methods The clinical data of 28 patients with para-superior sagittal sinus meningiomas in the middle cortex areas,who underwent surgery in our hospital from July 2000 to December 2005,were analyzed retrospectively. The clinical data included imaging data,the operative approach,and microsurgery method.Results According to Simpson's classification of glioma resection,of the 28 patients,17 received Grade Ⅰ resection of the tumors(60.7%),6 received Grade Ⅱresection(21.4%),3 received GradeⅢ resection(10.8%),and 2 received Grade Ⅳ resection(7.1%).No patients died from the operation.The following-up from 6 to 24 months showed that the meningiomas recurred in 4 patients.Conclusion The para-superior sagittal sinus meningiomas in the middle cortex areas can be totally resected by using microsurgical techniques,the measures including sufficient preoperative imaging evaluation,good protection of the vein of central sulcus,excellent management of superior sagittal sinus,and avoiding damage to functional cerebral cortex during the operations.It can significantly increase the percentage of total removal of the tumors,decrease postoperative complications,and improve survival outcome.
