Abstract

The human error problem can be viewed in two ways:the person approach and the system approach.To use the system approach,human errors are seen as consequences rather than causes,having their origins not so much in the perversity of human nature as inupstreamsystemic factors.Through the investigation of 4/28 railway accident,we can see some systemic factors of command delivery and check, control device,design that combined the human errors and caused the accident.
