The reliability research of 3D-CTA to the diagnosis and treatment of intracranial aneurysm

Abstract

Objective To study the 3D-CTA reliability on the diagnosis and treatment of intracranial aneurysm.Methods 240 patients with suspected to the ruptured aneurysms who displayed subarachnoid hemorrhage(SAH) on CT imaging were all examined with 3D-CTA.We operated directly on them who be found having aneurysms,the others who not be found having aneurysms were examined with 3D-CTA and DSA again.Then we analyzed the cerebral vascular showed by CTA and compared with DSA and operation.Results In all 240 cases,a total of 239 aneurysms were detected in 227cases.First detection rate was 94.6%(227/240).In the 13 cases who not be found having aneurysm in the first 3D-CTA,eight was found in the second 3D-CTA and DSA while five wasn't in both.Both the sensitivity and the specificity of 3D-CTA in the diagnosis of intracranial aneurysms was 100%,and the agreement rate in diagnosis was 100% also.The fake negative and positive rate was 0%(0/235).The important information such as the configuration of the aneurysmal sac and neck,calcification in the aneurysmal wall,and the aneurysms' anatomic relation with adjacent vessels and bone structures yielded from CTA was in perfectly accordant with what seen on operation.Conclusion Comparing with DSA,3D-CTA is a quick,reliable,and relatively noninvasive diagnostic tool for intracranial aneurysms.3D-CTA findings could provide the aneurysmal morphology in detail and play an important role in postoperative evaluation,follow-up intervention and epidemiology investigation.
