Abstract
Objective Aprospective study was conducted to study the influence of radiotherapy plus concurrent chemotherapy treatment on the longterm results for N2,N3 stage nasopharyngeal carcinoma. Methods 132cases diagnosed N2,N3stage nasopharyngeal carcinoma patients('92 stage system),were randomized into two groups,①Group (A66cases)Who received only radiotherapy with 68~86Gy(/6.8~9W) in nasopharynx and 64~80Gy/(6.4~8W)in the neck.②Group B(66cases) received the same radiotherapy as the group one and fore cycles of chemotherappy with cisplatin and 5-fluorouracil.The first cycle of chemotherapy was given one week before radiotherapy and repeat per 3 weeks .Results The complete response rates for the primary tumor in the group A and group B were 92.1% and 86.3%(P0.05).The remisson in the cervival lymph nods was revealed to be better in group A than that in group B,their CR rates were 89. 3% and 61.2% (P0.05).The primary tumor contral rates of group A and group B at 3-and 5-year were 76.8% to 45.4%(P0.05),and 65.9% to 47.2%(P0.05) respectively. The cervival lymph node control rates at 3-and 5-year were 65.1% to 41.9%(P0.05),and 59.3% to 37.2%(P0.05).The distant metastasis in group A was lower than that in group B.The 3-and 5-year were 22.5% and 43.8%(P0.05),and 27.1% to 48.9%(P0.05); The 3-and 5-year survival rates were 65.4% to 30.6%(P0.05),and 45.8% to 22.9%(P0.05).The nausea 、 vom and the oral mucosa erosion rates of group A was lower respectively. The side reaction can be endured. Conclusion Radiotherapy with chemotherapy can increase the survival rates and decrease the distant metasis in patients with N2,N3 stage nasopharyngeal carcinoma.
